You may complete a Request Form for PB’s participation at your health affair on-line.
Please also be advised:
PB’s participation is dependent on resources, scheduling and availability of staff. Completing the form does not guarantee the agency’s participation.
PB strongly encourages you to submit a completed Request Form at least 8 weeks prior to your event. Note that PB receives the highest number of requests between the months of May and September.
Do not advertise PB participation until you have received confirmation of the agency’s representation at your event from PB.
Please thoroughly complete the Request Form, as the information assists PB in determining its level of participation.
PB observes federal holidays; staff will not be able to participate in presentations on New Year’s Day, Martin Luther King’s Birthday, Washington’s Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veteran’s Day, Thanksgiving, and Christmas.
